Matthew and Jennifer sat anxiously in front of their laptop computer, listening to their fertility specialist counsel them on the results of their testing via video conference. They'd been trying their best to get pregnant for over a year and a half with no luck. Jen, especially, had spent countless hours online trying to find techniques to improve their chances of conceiving before they consulted Dr. Stephens. Although they feared the results, anything would be better than not knowing.
"So, guys, I've got good news and not-so-good news for you," the doctor began.
Jen was squeezing all the blood out of her husband's hand, waiting for the hammer to drop.
"The good news is that we couldn't find any obvious reason you can't get pregnant, Jennifer. That doesn't rule out every possible barrier, but we didn't find any problems within the scope of our tests, including the ultrasound examination."
Jen exhaled in relief, but wondered what the bad news could be. She had learned about one in five women have problems conceiving, so she had assumed she was the problem.
"Matthew, I don't ever like telling anyone this, but your fertility tests did reveal a problem. I'm very sorry to have to tell you, but our tests show you fall into the category of 'sterile'. You've heard of low sperm count, but our tests showed no viable sperm present. We can do more precise imaging and blood tests to make sure there's not a less obvious reason for this result, but our ultrasound didn't show any abnormal physiology or signs of disease."
The couple was devastated. Jen tried not to lose her composure, but she couldn't help but cry. Matt could never bear to see the love of his life this sad, so he couldn't hold back his tears either.
"I know this is very hard news to handle, but I want you to know there are still options for you to become pregnant, Jennifer. Please don't give up hope. Take all the time you need to digest this information, but please set up another video session with me if you want to discuss additional testing for Matthew and/or other ways to conceive."
"Thank you, Dr. Stephens," responded Matt, as Jen was in no state to speak.
As soon as the video chat ended, Jen ran to their bedroom, crying uncontrollably. Even though it was out of his control, Matt couldn't help but feel deeply guilty. Of all people, how could he be the one to crush one of her biggest dreams. He held her as she cried, but didn't know what to say except, "I'm so sorry." She knew she had to work hard not to cast blame on her husband. He was a good man, and she knew this wasn't his fault.
Over the next few days, the shock wore off and their despair somewhat turned to hope as they educated themselves on other options. They learned that if Matt was indeed sterile, a sperm donor would be required. They even read about how donors are screened by the clinic, how they are chosen anonymously by the recipient and the process of artificial insemination.
Although the procedure was less expensive than in-vitro fertilization, it could still become very expensive if it took several attempts to achieve pregnancy. At this point in their lives, they were both saddled with sizable student loan debt to pay off and had hoped to purchase their own home in a few years. This could easily put them farther in debt than they were comfortable with.
Once they lined up all their questions and were ready to discuss it further, they decided to meet with Dr. Stephens in person.
The more in-depth they discussed the artificial insemination process using frozen sperm from someone they'd never met, the more negative Jen became. She imagined herself as a cow in a head-catch, being bred by a cattle rancher with high-dollar bull semen before being zapped out into the corral with an electric cattle prod until repeating it the next month. It felt cold and inhuman. Matt didn't completely agree, but felt her opinion far outweighed his on this matter.
When the doctor asked what questions or concerns they had, Jen spoke first.
"Doctor, I'm concerned about the cost, but am I the only one who feels this process is very cold and impersonal? I know we don't really have other options, but this just isn't the way I dreamed about our child being conceived."
"So, the simpler issue is the cost. It's mostly dependent on how many attempts it takes to be successful. The best-case scenario is that you conceive on the first attempt, which would keep the total cost around twenty thousand. The more inseminations required, the higher the cost. It would be up to you guys as to how many times you'd want to try if we weren't successful," Dr. Stephens explained. "I've never had anyone spend more than about forty thousand trying."
The couple tried to play it cool upon hearing the cost, but neither had great poker faces. Although they both had decent jobs, Jen would be out of work for at least a few weeks after the delivery and they certainly didn't have that kind of cash saved. They had health insurance, but it didn't cover any portion of these costs beyond their diagnostic tests.
The doctor continued, "And no, you're certainly not the only one who isn't a fan of the process. We actually recommend our patients meet with a mental health counselor before the procedure because of those and many other emotional and ethical concerns. It's not something you should take lightly."
Jennifer continued, "But you're saying that's really our only option if I want to be pregnant, aren't you?"
Dr. Stephens paused momentarily before adding, "Well, it's not the only option, but very few patients opt for the alternative because of the complicated emotional aspects."
The couple looked at her, obviously not sure what she was talking about.
